Patient Journey in an Integrated Clinic: Registration to Follow-Up

A patient's visit to an integrated clinic involves reception, the doctor, the pharmacy, the lab and billing. Each step depends on the one before. When information moves smoothly between them, the patient waits less and the clinic makes fewer mistakes. This guide walks through the journey and the checks at each step.

Step 1: Registration and check-in

Reception finds or creates the patient record, checks contact details and records the reason for the visit. A clear check-in lets the doctor see who is waiting and why, without asking the patient to repeat the story.

Step 2: Triage and vitals

A nurse or assistant records vitals and any urgent concerns. These should appear on the doctor's screen with the time they were taken, so the doctor can see changes over time.

Step 3: Consultation

The doctor reviews the history, examines the patient, records a diagnosis and plan, and decides on prescriptions, lab tests or referrals. Each decision should be recorded against the visit, not in a separate note.

Step 4: Pharmacy

Prescriptions reach the pharmacy as soon as the doctor signs them. The pharmacist checks the prescription, dispenses from a batch, and records the sale. Any substitution or part-dispensing is recorded against the prescription.

Step 5: Lab

Lab orders are sent with the clinical reason, collected and tracked, and results return to the patient's record. The doctor is told when results are ready, and can review them before the next visit.

Step 6: Billing

Services from the consultation, pharmacy and lab are captured at the visit, and the patient receives one bill or separate bills, as the clinic's rules require. Payments are recorded against the right bill.

Step 7: Follow-up

The clinic records the follow-up date, sends reminders where consent allows, and reviews results and missed appointments. Follow-up is where chronic care is kept on track.

Where information is most often lost

Between reception and the doctor, when the reason for visit is not recorded
Between the doctor and the pharmacy, when prescriptions are re-typed
Between the lab and the doctor, when results are not flagged
Between the visit and billing, when services are not captured

Questions to walk through with your team

At which step does a patient wait longest, and why?
Where do staff write something down that is entered again later?
How does a doctor know that a result or a prescription has been received?
Which step causes the most calls or complaints?

Frequently asked questions

Can a small clinic follow this journey?

Yes. The steps are the same. A small clinic may combine some roles, but the information should still move between them in the same order.

Where should we start improving the journey?

Start with the step where patients wait longest, or where most information is re-typed. Fix one step, measure the result, then move on.

Does the patient need to repeat information?

Ideally no. A shared record means the patient gives their details once, and staff check and update them when needed.
